This app incorporates lab data from over 15m patient records. It covers list of bacteria in urine, blood, or skin for communities across the nation. Antibiotic drug options are provided by organism susceptibility. Warning and dosing information are covered also. The app has a clean design but does require iOS 7 to run.

Bugs + Drugs is free at the time of this writing. It is a decent reference app but for a more complete drug reference, you should probably check the Epocrates app.
